DAC Appui Santé Lille Agglo is a French nonprofit association supporting healthcare, medico-social and social professionals who face complex care situations, particularly when a person’s care pathway is disrupted or at risk of disruption. Operating across 38 communes in the Lille area, it provides a single point of contact to help professionals understand available services, find appropriate resources and connect with local providers.

Its main services include information, guidance and advice, as well as support coordinating complex health pathways in collaboration with other stakeholders and the patient’s primary care physician. The team also offers expertise in geriatrics and palliative care, including support for care at home. Beyond individual cases, the association contributes to local coordination, shares good professional practices and identifies gaps in care pathways to inform partners and authorities.

DAC Appui Santé Lille Agglo was formed by bringing together three services focused on complex situations involving older people, geriatric care at home and palliative care for adults. Professionals can request support by phone or email, Monday to Friday.
