Aalto Home Care is a Minneapolis-based, 245D-licensed provider serving adults ages 18 to 55 across the Twin Cities and surrounding metro area. The organization supports people with developmental disabilities, brain injuries, physical disabilities, and mental health conditions, offering culturally responsive care shaped around each individual’s needs, routines, language, and family. Its staff also work alongside clients’ wider care teams, including doctors, nurses, mental health providers, social workers, and case managers.

Services include Integrated Community Supports, with staff available in the building where clients live; in-home assistance with daily routines and skill building; homemaking support such as cleaning, laundry, and meal preparation; and awake overnight supervision. Aalto Home Care develops personalized plans to help clients manage everyday activities and build skills at home and in the community. Payment options include Minnesota waiver and county funding, private pay, and plans from Medica, UnitedHealthcare, and Blue Cross and Blue Shield of Minnesota.
