Buxbaum Medical is a Houston family medicine practice providing board-certified care in its office and across assisted living, independent living, nursing home, and memory care communities. Serving Houston families since 2003, the practice is built around continuity: patients can receive care from the same provider team in different settings, with medical records and care plans following them.
