Dr. Carrie R. Kubota & Associates is an optometry practice serving the Simi Valley community since 1994. With more than 30 years of eye care experience, the practice focuses on comprehensive, personalized care in a friendly, family-oriented environment, using modern technology to support patients’ eye health and vision needs.

Services include comprehensive eye examinations, pediatric eye care, specialty contact lens fitting, dry eye treatment, and myopia management. The practice also offers customized eyewear, with a selection of fashionable frames, and works to match lenses and contacts to each patient’s needs. For dry eye, care may include Forma-I radiofrequency treatments, as noted by a patient testimonial.

Led by Dr. Kubota, the practice emphasizes thorough examinations, clear explanations, and attentive service. Patients can request appointments and find information about payment and insurance through the practice’s patient resources. Dr. Kubota and her team aim to provide individualized care for patients at different stages of life, from children’s developing vision to adults’ ongoing eye health and eyewear needs.
