Center for Elders’ Independence (CEI) is a nonprofit healthcare provider serving at-risk people in California’s East Bay. Its mission is to help participants live as independently and safely as possible while honoring their needs, choices, and life experiences. CEI offers two programs: PACE, a personalized, comprehensive care program for adults aged 55 and older with multiple health challenges, and the Home and Community-Based Alternatives (HCBA) Waiver Program, which coordinates medical services for vulnerable people of all ages who need support to live at home.
PACE services bring together medical care and care coordination, with teams developing personalized plans and managing referrals, prescriptions, paperwork, and appointment transportation. Participants can also access home care for daily tasks, eye and dental care, physical, occupational, and speech therapy, nutritious meals, and social activities. CEI provides transportation to its centers, medical appointments, and program outings. The HCBA program supports eligible individuals in community settings and requires Medi-Cal eligibility. Program costs vary by coverage and eligibility; many participants qualify for full or partial coverage.
