About Essence Hospice Care

We provide hospice care in various settings, including patients' homes, nursing homes, assisted living communities, residential care facilities, and hospitals. At Essence Hospice, we believe in holistic care, which is why we offer a range of services, including hospice at home, condition-specific care, bereavement and grief care, 24/7 telecare, care for veterans, home medical equipment, music therapy, and pet visits. Our care services are coordinated with the patient's physician and provided through regularly scheduled visits.
In case of any emergencies, we are available 24/7 to respond to our patients' needs. In addition to traditional hospice care, Essence Hospice also offers palliative care services to help patients and families achieve comfort and symptom relief. We provide four levels of care, including routine home care, respite care, general inpatient care, and continuous care.
We understand that grief is a natural part of losing a loved one, which is why we provide consistent and continuing bereavement support to families, friends, and staff for up to 13 months following a death. Our grief and bereavement services include home visits, telephone support, memorial services, and community resource referrals. We offer a wide range of medical equipment, such as oxygen devices, nebulizers, hospital beds, wheelchairs, and pressure-relief mattresses, to ensure a seamless transition from hospital to home.
