Evolution Weight Loss & Hormones is a functional medicine practice serving patients in Clearwater and Lutz, Florida, with services also available nationwide. The brand focuses on personalized care, aiming to help people manage chronic illness, address underlying health concerns, and pursue improved wellbeing. Its approach begins with a one-on-one consultation, followed by extensive bloodwork to assess individual needs. The team then discusses a tailored plan, which may include supplements, and provides ongoing support to help patients manage and maintain their health.

Services include medical weight loss, including semaglutide and tirzepatide programs, hormone therapy, genomic methylation panel testing, and sexual health care. The practice also offers functional medicine support for conditions and concerns such as POTS, PCOS, endometriosis, adrenal fatigue, fibromyalgia, chronic conditions, food allergies, mental health, and leaky gut. Additional offerings listed include peptide therapy, acoustic wave therapy, concierge medicine, and Vita Trim supplements. Evolution describes its care as a personalized pathway rather than a one-size-fits-all plan, with the goal of helping patients work toward better health based on their individual circumstances.
