Evolve Healthcare Group is a mobile Occupational Therapy service supporting NDIS and My Aged Care participants in Melbourne, Victoria, with telehealth services available across Australia. Founded by Senior Occupational Therapist Taylah Godeassi, the group helps people adapt to life’s challenges and work toward greater independence and wellbeing. Its senior team provides assessment services for adults and children, including functional capacity assessments and reports, NDIS access reports, home and living assessments for SIL, SDA and ILO arrangements, assistive technology prescriptions and reviews, home modification assessments, vehicle modification assessments, and Home Care Package assessments.

Evolve also offers ongoing therapy and capacity-building services for participants aged 15 and older, with a focus on confidence, independence and quality of life. The team draws on experience with NDIS and My Aged Care funding frameworks and advocates for outcomes aligned with each person’s needs and goals. Its services also support older people to maintain safety and independence at home and in the community.
