Donald Fowler, a devoted family man, active community supporter, and dedicated practitioner, who served clients in Rock and Green Counties for over 30 years. Thanks to the generosity of the Fowler family, we have been able to continue Dr. Fowler's legacy and make dental care more accessible in Green County.

Since our inception in 2009, we have made significant strides in expanding our services and impact. Initially operating as a pediatric dental clinic, we have evolved to serve individuals of all ages residing in Green or Lafayette counties, Wisconsin, who have ForwardHealth insurance. We have also extended our hours to accommodate our patients, now open Monday-Thursday from 8:00 am to 5:00 pm by appointment.
