Hamaspik Choice is a Managed Long-Term Care (MLTC) plan serving eligible Medicaid members in New York. Its purpose is to coordinate long-term care so members can continue living in their homes and communities. The plan supports people aged 18 and older who meet Medicaid and care-need requirements and reside in a covered county. Its service area includes the Bronx, Dutchess, Kings, Nassau, New York, Orange, Putnam, Queens, Richmond, Rockland, Suffolk, Sullivan, and Ulster counties.
Hamaspik Choice develops individual care plans through nurse and social work care managers, who coordinate with members’ physicians and connect them with a network of local providers. Covered services include personal care aides, skilled and private-duty nursing, home health aides, physical, occupational, and speech therapy, adult day health care, dental and hearing services, durable medical equipment, medical supplies, transportation, and home-delivered meals. Other supports include nutrition counseling, social services, personal emergency response systems, and home modifications.
The organization also offers enrollment guidance, member resources, provider information, and assistance with grievances and appeals.
