Health Opportunities Consortium (HOC) is a North Carolina provider of health-related social support services focused on advancing equity and improving outcomes for Medicaid beneficiaries. With more than five years of experience delivering nonmedical services under the North Carolina Medicaid 1115 Waiver, HOC works with Medicaid programs, health systems, community organizations, and local agencies to address social determinants of health and strengthen community support.

Its services include housing navigation, eviction prevention, short-term and post-hospitalization housing, and assistance with moving costs and essential utilities. HOC also supports access to nutritious food through healthy meal delivery, food boxes, fruit and vegetable prescriptions, nutrition education, and diabetes prevention. Other offerings include non-emergency transportation, vehicle repair assistance, health-related case management, care coordination, and help accessing legal support.

The organization also provides services addressing interpersonal violence and trauma, including case management, referrals, and therapy, alongside home safety assessments, accessibility modifications, and home goods. Through its work connected to North Carolina’s Healthy Opportunities Pilot, HOC aims to reduce barriers to care and help individuals and families achieve better health and well-being.
