Health Support Care

Health Support Care is a Registered Nurse-founded and nurse-led provider delivering disability, aged care and clinical support across Brisbane, the Gold Coast, Ipswich and Caboolture. Its team brings experience in acute nursing, aged and disability care, gastroenterology, endoscopy and dysphagia care to people’s homes and community settings. Support is tailored to each person’s health needs, routines, goals and preferences, with an emphasis on dignity and personal attention.

Services range from everyday living and disability support to complex, high-acuity nursing care. Specialist services include feeding tube care for PEG, PEG-J, NGT and NJT tubes; tracheostomy and ventilator-dependent support; dysphagia care; medication management; and pre- and post-operative nursing at home. The provider also offers aged care and DVA services, and supports participants using agency-managed, plan-managed or self-managed NDIS plans. Health Support Care is registered with the NDIS Quality and Safeguards Commission and welcomes referrals from families, hospitals, support coordinators and plan managers. It also offers guidance to people who are new to the NDIS, helping them understand available support options.
