The Institute for Safe Medication Practices Canada (ISMP Canada) is a national partner dedicated to strengthening medication safety across Canada. It works with health care providers, organizations, consumers, and caregivers to identify medication-related risks and develop practical ways to prevent harm. The organization collects and analyzes reports of medication errors and other incidents, then shares evidence-informed recommendations to support safer care.

ISMP Canada offers safety bulletins, consumer newsletters, educational resources, workshops, webinars, and consulting services. Its resources address medication safety in settings including hospitals, community pharmacies, long-term care, and palliative care at home. Practitioners, community pharmacies, consumers, and caregivers can submit reports to support shared learning and improvement. Through the Community Pharmacy Reporting and Learning program, participating pharmacies contribute anonymized incident reports to a national repository.

ISMP Canada also supports the Canadian Medication Safety Network, connecting health care providers and consumers to discuss medication safety needs and opportunities. Its work includes partnerships that help organizations implement, sustain, and evaluate improvements, as well as self-assessment tools and guidance for consumers preparing to discuss medications with practitioners.
