Kay's Konnections

Kay’s Konnections provides person-centered support for individuals of all abilities, with a focus on inclusion, dignity, and meaningful community connections. The organization aims to empower people through personalized care, advocacy, collaboration, and education, while working alongside families, guardians, providers, and community partners. Its approach is grounded in compassion and respect, meeting each person where they are and offering support when it is needed.

Services include Daily Supported Community Living (SCL) in small, family-style group homes, with no more than four individuals per home. These homes are staffed around the clock by trained professionals and are designed to provide a safe, welcoming environment. Kay’s Konnections also offers host home arrangements, where individuals receive individualized care in the home of a qualified provider. The organization currently accepts ID and BI waivers and works with managed care organizations including Wellpoint, Molina, and Iowa Total Care. Referrals are welcomed from managed care organizations, and prospective applicants can submit an application directly.
