Life Connections Support Services

Life Connections Support Services is a disability support provider based in Moreton Bay City, Queensland, focused on helping people with disabilities build fulfilling, independent lives. Founded by Amber, an advocate with experience in disability and human services, the organisation offers personalised support shaped around each person’s strengths, needs and goals. Its services include assistance with daily activities such as personal care, mobility, shopping, cooking and housework; emotional support; help maintaining connections with family, friends and community services; goal setting; and support to develop or maintain independence.

Assistance can be delivered in clients’ homes, residential and community settings, and in connection with clinical, medical or hospital appointments. Life Connections works collaboratively with individuals and families to create tailored care plans and promote inclusion and community participation. The organisation also conducts risk assessments and documents shifts, including progress notes, incidents, risks and goal progress, using the Shiftcare app.
