Madrone Care Network is a community-based specialty palliative care program serving people in Del Norte, Humboldt, Mendocino, Lake, Santa Clara, Shasta, Siskiyou, and Trinity Counties. Its care is delivered where patients live and receive support: in their homes or communities, through in-person visits or video conferences. The program’s goal is to reduce stress and improve quality of life while helping patients manage complex medical issues.

Madrone Care Network provides 24/7 care through a specialty-trained, compassionate team of physicians, physician assistants, nurses, social workers, chaplains, community health workers, and patient care coordinators. Team members work collaboratively with each patient’s existing medical team, adding support alongside their current care. This coordinated approach brings together clinical and community-based roles to help address patients’ needs.
