It is a free online archive and distribution server that specializes in hosting complete but unpublished manuscripts, also known as preprints. These preprints are preliminary reports of work conducted in the medical, clinical, and related health sciences that have not yet undergone peer review. It is important to note that they should not be relied upon for clinical practice or health-related behavior, and should not be reported as established information in news media.

The platform allows researchers to share, comment, and receive feedback on their work before it is published in a journal. medRxiv accepts a variety of research articles such as systematic reviews, meta-analyses, clinical research design protocols, and data articles, as long as they adhere to scientific principles. As a non-profit community resource, medRxiv is not affiliated with any particular publisher or journal. Articles posted on the platform undergo a basic screening process for offensive and non-scientific content, and are checked for plagiarism. Although they are not peer-reviewed, edited, or typeset before being posted online, they receive a digital object identifier (DOI) and are indexed by numerous search engines and third-party services.
