Onze doelstelling is om rond 2030 een veilige, huiselijke en rustige woon- en leefomgeving in Zwolle te realiseren, voor 27 mensen met een verstandelijke en/of ernstig meervoudige beperking met een zorgindicatie VG 5 t/m 8. Het Mooi Leven Huis Zwolle wordt een thuis waarbij mooi leven en passende zorg ontstaan door evenwaardige samenwerking tussen zorgprofessional, de bewoner en zijn of haar familie, diens netwerk en de omgeving. Mooi leven betekent een zinvolle en uitdagende dagbesteding (dit kan ook arbeidsmatig zijn). Dit wordt geboden, niet los van de omgeving, maar met een actieve betrokkenheid van het sociale netwerk en de leefomgeving. De professionele zorg wordt gegeven door mensen die zich betrokken voelen bij de bewoners en hun netwerk. De zorg beweegt mee in het leefritme van de bewoner en hun netwerk. Gezamenlijk stemmen zij de zorg en het leven van de bewoner af. We doen het samen, inclusief en bundelen onze krachten binnen en buiten ons Mooi Leven Huis Zwolle.

About Mooi Leven Huis Zwolle
Mooi Leven Huis Zwolle is a dedicated organization focused on creating an inclusive and nurturing environment for children and young adults with intellectual and severe multiple disabilities. With the motto "A life as we would want to live it," the initiative champions a collaborative approach to care, involving not only healthcare professionals but also families and the local community.
Set to establish a new home in Zwolle, Mooi Leven Huis aims to provide residents with a sense of belonging while offering professional care tailored to their needs. Each living group will consist of six to eight residents, carefully matched for compatibility in behavior and character, to foster a harmonious living dynamic.
Families play an integral role by actively participating in activities and maintaining connections, ensuring that the essence of family life continues even away from home. Mooi Leven Huis Zwolle envisions building a strong support network for its residents, allowing them to thrive in an environment that embraces their individuality.
