Veteran’s Health and Wellness Foundation (VHWF) is a non-profit 501c3 charitable entity established by military Veterans with expertise in the area where health, wellness and healthcare delivery intersects with the social determinants of health and military culture. VHWF exists for the express purpose of addressing systemic inequities regarding access to and quality of care for Veterans and their families. The Veteran Medical Neighborhood Consortium (VMNC) is a framework for structured bidirectional and cross functional relationships and partnerships (clinical and non-clinical) that create a world where ALL Veterans and their families live healthy and well lives undiminished by their service related health concerns. Our Mission is to build, educate, and maintain resource networks to support the complex physical, social, and emotional needs of Veteran communities across the nation. Veterans Health and Wellness Foundation (VHWF) is a nonprofit organization dedicated to addressing systemic inequities in quality care for Veterans and their families. Established by military Veterans with expertise in health, wellness, and healthcare delivery, VHWF focuses on providing access to and improving the quality of care for Veterans nationwide. Since 2014, VHWF has positively impacted over 40,000 Veterans and their families, supporting their mental and physical healthcare needs.

They envision communities that have the knowledge and capacity to identify and address service-related health challenges while supporting and empowering Veterans and their families. To achieve these goals, VHWF offers flagship programs such as community health and wellness events, Veteran and family-centered health education forums, and healthcare education on Veteran culture and military environments.
These programs provide free healthcare screenings, vaccinations, benefits navigation, medical appointments, and informative events to increase awareness and knowledge about the unique healthcare needs of Veterans. VHWF also offers a resource search center technology platform to connect Veterans and their families with verified social care programs at the local level.
