About NDPAP
NDPAP, the National Directory of Post-Acute Providers, is a free online directory that helps patients, families, and care teams find and compare post-acute care services across the United States. Its listings cover home health agencies, hospice providers, durable medical equipment suppliers, and pharmacies, searchable by city, ZIP code, or location. Built from public Medicare data and provider updates, the directory provides real phone numbers and addresses so users can contact providers directly without an account, lead forms, or fees.
Search filters include practical referral needs such as accepting new patients, language options, and weekend or after-hours coverage. Users can compare up to three providers side by side, while families, case managers, and discharge planners can access plain-language care guides, checklists, and provider-selection resources. NDPAP updates its public data weekly, and verified providers can manage listing details in real time.
