About NW Primary Care
Northwest Primary Care (NWPC) is a physician-owned healthcare provider serving the southeast Portland metropolitan area for more than 50 years. Its Medical Home approach emphasizes personalized attention, coordinated care, and strong relationships between patients and their care teams. Providers work with each patient to develop an individual care plan, support communication across specialties, and help patients take an active role in managing their health.
