Alliance for Pediatric Medical Homes

The Alliance for Pediatric Medical Homes (APMH) is an accreditation alliance focused on helping pediatric practices achieve Patient-Centered Medical Home (PCMH) recognition through requirements designed specifically for children’s healthcare. It offers a collaborative, streamlined alternative to general PCMH programs, aiming to reduce unnecessary operational work while supporting efficient practices and healthier children. APMH’s core requirements cover practice fundamentals, access and timely communication, identifying patient needs and appropriate care options, care coordination for complex patients, and performance measurement and quality improvement.

Participating practices receive a dedicated transformation coach, and the program is designed to work with existing tools; an electronic medical or health record system is required. APMH also offers optional specialized tracks that recognize achievements in human resources and staff training, behavioral and mental health support, and attention to social determinants of health and disparities. Its Preferred Partner Program allows companies and vendors with pediatric-specific products or services to apply for pre-validation, helping their clients pursue APMH accreditation
