Realigned Coordination

Realigned Coordination is an Adelaide, South Australia-based provider of NDIS support coordination and psychosocial recovery coaching, led by Nick Podmore. The service helps participants understand and use their NDIS plans, connect with suitable providers, and advocate when supports are not working. Nick offers Support Coordination Level 2 for hands-on assistance, Specialist Support Coordination Level 3 for participants with complex needs, and psychosocial recovery coaching that supports individual recovery journeys. Capacity-building assistance is also available for self-management and plan management, including understanding plan documents and communicating with providers.

The approach emphasizes continuity, plain-language explanations of funding and options, and support informed by lived experience of disability. Realigned Coordination describes its practice as neuro- and queer-affirming. Additional services include risk assessment and management planning, individualized support plans, help finding community and mainstream programs, exploring alternatives to conventional supports, and coordinating across health, disability, and other services. Participants can begin with a no-pressure conversation about their goals and plan, followed by provider connections, service agreements, and ongoing check-ins.
