The Regional Geriatric Program (RGP) of Toronto is a trusted leader in the care of seniors with frailty, with over 30 years of experience. Their team consists of experts in geriatrics, gerontology, adult education, quality improvement, knowledge translation, implementation science, and information technology. The RGP focuses on achieving better health outcomes for older adults living with frailty throughout the healthcare system.
They support a network of specialized geriatric service teams across 23 organizations, providing coordinated and high-quality care by interprofessional teams of highly skilled health professionals. The RGP also collaborates with healthcare providers, older adults, caregivers, and system partners to advocate for health system change that reflects the unique needs of seniors with frailty. Additionally, the RGP builds capacity for Senior Friendly Care TM by providing resources, support, and key resources to enhance the capacity of health service providers and older adults and caregivers to care for themselves.
As a not-for-profit corporation, the RGP operates under the governance of a Board of Directors and is funded by the Toronto Central Local Health Integration Network.
