Second Chance for Life Foundation (SCFLF) supports heart transplant and left ventricular assist device (LVAD) patients and their families during the challenges of treatment and recovery at University of Minnesota Medical Facilities. Working in partnership with the University of Minnesota Medical Center and U of M Masonic Children’s Hospital, the foundation provides mental, emotional, physical, and financial assistance to patients and caregivers.

SCFLF helps people navigate the transplant and LVAD journey through patient and caregiver resources, support groups, educational news, and opportunities to connect with others facing similar experiences. Its community includes patients, families, volunteers, board members, and transplant recipients. The foundation also shares patient stories and offers events that bring its community together, including celebrations and an annual golf tournament. Visitors can explore its photo gallery, join the community for updates, donate, or shop for branded merchandise. With a mission centered on practical support and connection, Second Chance for Life Foundation aims to accompany patients and caregivers through difficult times and the milestones of life after transplantation or LVAD implantation.
