Silverstar

Silver Star is a Lubbock, Texas-based care program focused on helping people with health challenges remain independent, live at home, and participate in their community. Its approach emphasizes small, personal service and compassionate support for both participants and their families. The program is designed for people who might otherwise need nursing home care, providing personalized healthcare and assistance intended to enhance quality of life and support caregivers through training, support groups, and respite opportunities.

With approval from its Interdisciplinary Team, participants may receive comprehensive medical services, including physician care, prescription medications, hospitalizations, skilled nursing, social services, home health care, and transportation with an escort to medical appointments. Silver Star also offers regular access to its Day Health and Wellness Center, which includes a staffed medical clinic, therapy services, social service referrals, exercise equipment, recreational activities, an outdoor patio and gardening area, meals, and transportation to and from the center. Prior authorization is required for most services, and participants may be financially responsible for unauthorized care outside the PACE agreement.
