Sourcewise

Sourcewise is an independent nonprofit serving older adults, adults with disabilities, and caregivers in Santa Clara County. A partner in aging services since 1973, it helps community members navigate complex systems and find support suited to their needs. As a central access point for local aging and disability services, Sourcewise connects people with community providers and coordinates assistance across nutrition, health care, caregiving, housing, transportation, employment, and technology.

Its programs include Medicare counseling and enrollment guidance through the Health Insurance Counseling and Advocacy Program, care management, caregiver support, Meals on Wheels, senior employment services, digital access programs, and the Senior Farmers’ Market Nutrition Program. Sourcewise also offers services such as Enhanced Care Management, the Multipurpose Senior Services Program, South County services, senior transportation in South County, and home modifications for older adults. Its resource connection helps people locate a broad range of community support.

Beyond direct services, Sourcewise develops the local Area Plan on Aging and advocates for policy improvements to expand equitable access.
