St. Paul’s PACE is a San Diego-based program of St. Paul’s Senior Services, an organization serving older adults since 1960. Since 2008, its Program of All-Inclusive Care for the Elderly has supported thousands of local seniors with coordinated healthcare and practical assistance designed to help them remain safely at home. The managed care program is for people aged 55 and older who live in San Diego County, meet California’s level-of-care requirements, and can live in the community with PACE support. Services are available only to enrolled participants.
Care includes primary and geriatric medical services, medication management, specialty care, behavioral health, physical therapy, and telehealth appointments. Participants may also receive in-home care following an assessment, help from social workers, and door-to-door transportation to medical appointments. At its day centers, St. Paul’s PACE offers social and dietary support in a secure setting, with meals and opportunities for connection. The organization serves communities including San Diego, Chula Vista, El Cajon, Mission Valley, and North County, with local clinics and centers.
