About Summit Elder Care
Summit ElderCare is a Program of All-Inclusive Care for the Elderly (PACE) offered by Fallon Health in Worcester, Massachusetts. It helps older adults with advanced health care needs live safely in their homes and communities, offering an alternative to nursing home care when participants can remain safely in the community with appropriate services. The program provides individualized medical care, medication coverage, social support, in-home care, health insurance and memory care, among other services.
Support may be suited to people who need assistance with daily activities or skilled services such as nursing, physical therapy or occupational therapy. Summit ElderCare also supports caregivers who need help providing care, and offers resources for current participants, including documents, monthly activity calendars and caregiver support groups. Care and services are coordinated through the PACE interdisciplinary team; services generally must be provided or authorized by that team, except in emergencies.
