SunnyvaleWellnessFoundation

Sunnyvale Wellness Foundation is a nonprofit community health organization providing evidence-based medical care and addressing social factors that contribute to healthcare disparities. Its mission is to deliver compassionate, quality healthcare and essential resources to underserved communities, helping people lead healthier, more productive lives. Services include adult primary care, with preventive and ongoing support; pediatric care for infants, children, and adolescents; and behavioral health support for emotional, psychological, and substance-use challenges.

The foundation also offers women’s health services, medical outreach, screenings, and health education. Through workshops, seminars, and community programs, it shares information on health literacy, prevention, nutrition, mental health, and chronic disease management. Its work spans local clinics and global operations, with listed locations in Northside, Pittsburgh, and Philadelphia’s Rittenhouse Square and Port Richmond neighborhoods.

Care is delivered by a multidisciplinary team that includes physicians, nurse practitioners, physical and occupational therapists, and support staff. The organization invites individuals to contribute through donations, volunteering, or partnerships, and reports that many outreach and screening programs are provided at no cost
