The Carpenter Health Network

The Carpenter Health Network is a network of post-acute care providers serving patients and families across Louisiana, Texas, Arkansas, Mississippi, Alabama, and Florida. Its stated mission is to provide the right level of care at the right time, combining inpatient and in-home services across a broad continuum of needs.

The network’s services include primary care through Homedica House Calls; home health, skilled nursing, and therapy through STAT Home Health; and palliative care through AIM Palliative Home Health. St. Joseph Hospice provides hospice care, including crisis support, bereavement services, and access to The Carpenter House, an inpatient hospice unit that also offers respite stays for caregivers. Sage provides inpatient and outpatient rehabilitation, therapy, wound care, and long-term acute care hospital services. The network also includes nursing and rehabilitation facilities offering skilled nursing, memory care, psychiatric services, wound care, therapy, and recreational activities, as well as companion home services and private-duty care. Healthcare providers can access referral information, while patients and families can find service locations and request information through the network.
