The Coordination Hub

The Coordination Hub is an Australian provider of NDIS support coordination and psychosocial recovery services, based in Austinmer, New South Wales. Its mission is to empower participants to lead self-determined, fulfilling lives by championing dignity, inclusion, choice and independence. The organisation focuses exclusively on these services, aiming to offer impartial guidance that helps people find supports suited to their needs.

Through NDIS Support Coordination, the team helps participants understand and use their plans, connect with service providers and community, mainstream and government services, and build skills for greater independence and participation. Coordinators can also assist with negotiating provider services and preparing for plan reviews. Specialist Support Coordination is available for participants with high or complex needs, helping address barriers and coordinate consistent support across areas such as health, education and justice. Psychosocial Recovery Coaching provides personalised recovery planning and ongoing support, working collaboratively with participants, families, carers and other providers.
