Thrive South Side connects Chicago’s South Side residents with healthcare and practical support tailored to their needs and circumstances. Formerly known as South Side Healthy Community Organization, it focuses on uninsured people and Medicaid patients living in 15 South Side ZIP codes. Its services are available to eligible residents ages 18 to 64, with people outside the listed ZIP codes encouraged to contact the organization to explore options.

Thrive South Side uses community health workers and nurse care coordinators to help patients navigate care. The process begins with an assessment of medical and social needs, followed by a care plan, connections to providers and resources, and ongoing check-ins. Health services include primary and preventive care, specialist connections, maternal health support, heart health education and monitoring, and help managing chronic conditions such as diabetes, hypertension, and asthma.

The organization also connects people to transportation for medical appointments, housing and utility resources, food access, and employment and job training.
