Together Aid

Together Aid Community & Nursing Services is an NDIS provider based in Sunshine, supporting participants in Melbourne’s west and other non-regional areas of Victoria. The organisation provides practical, person-centred assistance for people with complex or changing needs, including psychosocial disability and mental health needs, neurodivergence, physical disability, a forensic history, or alcohol and other drug support needs. Its approach is informed by nursing and mental health experience, with support shaped around each person’s goals, preferences and routines.

Services include personal care and daily living assistance, household tasks, shared living support, community nursing, high-intensity personal support, behaviour support, accommodation and tenancy assistance, and Specialist Disability Accommodation. Participants can also access community participation, group activities, travel and transport, employment support, life skills development and support coordination. Together Aid works with participants’ families, treating teams and support coordinators, with consent, to help plan coordinated care. The organisation also supports hospital-to-home transitions, preparing with care teams and providing agreed assistance after discharge, including nursing support when included in a participant’s plan.
