Touchstone Living Care

Touchstone Living Care of West Bend is a Wisconsin-licensed Community Based Residential Facility providing assisted care in a small, homelike setting. Located in West Bend, the residence serves people with traumatic brain injuries, developmental and physical disabilities, Alzheimer’s disease, and dementia. Its country-like surroundings and campus-style environment are designed to support comfortable daily living and a sense of community.

Care is provided around the clock by certified staff trained to support residents’ needs. Touchstone emphasizes resident-centered care, developing an Individual Service Plan for each person and working with them toward personal goals. Its Class CNA licensing allows it to serve ambulatory, semi-ambulatory, and non-ambulatory residents. With a maximum capacity of eight, the facility offers a small-scale setting where staff can provide attentive, one-to-one support.

The community also encourages group activities and the continuation of simple family traditions, helping residents feel included in the Touchstone family. Touchstone Living Care of West Bend partners with sister facilities in Mayville, Jefferson, and Johnson Creek, including a memory care location.
