About Transitions Clinic
Transitions Clinic Network (TCN) is a national organization dedicated to dismantling the injustices caused by mass incarceration and addressing racial health and economic disparities. By focusing on the health and social determinants of individuals returning from incarceration, TCN aims to transform health systems and improve outcomes for impacted communities.
TCN implements their unique model of health system transformation, which involves training and providing technical assistance to community-based primary care clinics. They also hire and train formerly incarcerated individuals as community health workers (CHWs) who work within primary care teams to engage patients and bring about change from within the health systems.
Additionally, TCN trains health systems to adopt patient-centered policies and organizational practices that support employment opportunities for CHWs with incarceration histories. They collaborate with various stakeholders to promote healthcare access and equity, and offer community-led policy expertise and implementation support to strengthen initiatives for populations affected by the criminal legal system.
Through their innovative approach, TCN addresses the underlying inequities of race and place, leading to improved health and criminal-legal outcomes for patients.
