The Wellness Plan Medical Centers provides healthcare to Michigan residents, with a mission to make affordable, high-quality primary and preventive care available to people who may have limited access, including uninsured and underinsured patients. The organization has served its community for 45 years and supports patients through a patient-centered medical home model. In this approach, patients work one-on-one with a primary care provider who helps coordinate care with a broader medical team.

Services include family medicine, internal medicine, pediatrics, and OB/GYN care, along with dental services, pharmacy services, integrated behavioral health, and the Women, Infants and Children (WIC) program. Patients can also access telehealth, patient resources, and after-hours and walk-in information. Pharmacy delivery is available from the Gateway, Northwest, and East locations.
