YP Coordination

YP Coordination is an Australian organisation supporting people with disability to make the most of their National Disability Insurance Scheme (NDIS) plans. Founded by Ali and Fallon in response to a growing need on the Yorke Peninsula, the organisation focuses on trust, respectful relationships and helping participants feel heard and understood. Its work also reflects a broader commitment to inclusion, community connection and enabling people to pursue their goals.

YP Coordination provides NDIS Support Coordination, helping participants put their plans into action, make informed decisions, connect with suitable service providers, build independence and address challenges. It also offers one-to-one coaching to help people understand their plans and maximise their funding. These services are available to participants with plan-managed or self-managed plans; the organisation states that it is not currently a registered NDIS provider and cannot support agency-managed Support Coordination funding.

In addition, YP Coordination runs inclusive workshops and community activities, with multiple workshops planned each month.
